[PPEUM Clinic (hereinafter referred to as "the Company")] places a high priority on the protection of personal information.   • Mandatory Information: Name, resident registration number, address, phone number, mobile phone number, email address, marital status, source of referral, resident registration number (for foreign nationals)
  • Health Information: Medical history, family health history, and other health-related information deemed necessary by medical professionals

Credit Card Payments

  • Collected Information: Card issuer, card number, and other credit card payment authorization details

  • Personal information is collected through the website, written forms, fax, phone, consultation boards, and email.

 

Article 2: Purpose of Collecting and Using Personal Information

The Company utilizes the personal information collected for the following purposes:
All information provided by users is used solely for the purposes listed below. In the event of any changes to the purposes of use, prior consent will be obtained from the user.

  • Services for diagnosis and treatment
  • Identity verification procedures for medical consultations, tests, appointment inquiries, and treatment
  • Administrative services such as billing, payment, and refunds for medical fees
  • Issuance and delivery of medical expense statements, detailed statements, certificates, prescriptions, supplies, and results
  • Commissioned tests online/offline and referrals for external testing
  • Securing communication channels to address complaints and grievances
  • Legal and administrative responses and actions for quality management and hospital operations
  • Minimum analytical data required for education and research
  • Collection and use for marketing purposes, including providing information about branch-specific events, new services, and updates via SNS, text messages, email, KakaoTalk (notification messages, branded messages), and phone calls
  • Providing updates on medical, academic, and hospital information

 

Article 3: Provision and Sharing of Personal Information

Purpose of Personal Information Use by Recipients

  • To verify patients and manage tasks related to appointment scheduling, cancellations, and other processes
  • To provide information about the Company’s services, new offerings, and event details
  • To send mobile notifications regarding consultations, appointments, and related services
  • Personal Information Provided to Recipient: Name, ID, password, gender, mobile phone number, email, residential area

Retention and Usage Period of Personal Information by Recipients

  • The retention period of personal information is the same as that of the collecting organization. However, in the case of contract termination (or cancellation) with the collecting organization, the retention period is limited accordingly.
  • Users have the right to refuse consent. However, refusal to provide consent may result in the inability to schedule medical appointments, which could inconvenience users and affect satisfaction.

Period of Information Retention

  • The retention period of personal information is the same as that of the information collection organization.
    However, in the event of termination of contract with a personal information collection institution, the patient has the right to refuse consent.
  • If consent is refused, the patient may not be able to make an appointment and the patient's convenience and satisfaction may be affected.

 

Article 4: Provision of Personal Information to Third Parties

The Company processes personal information solely within the scope outlined in "Article 1: Collection and Retention of Personal Information" and provides personal information to third parties only with the user's prior consent or as required by applicable laws.

Article 6: Retention and Use Period of Personal Information

In principle, personal information is destroyed without delay once the purpose of its collection and use has been achieved. However, the following information is retained for the specified period for the reasons stated below:

  • Retained Items: Name, gender, login ID, password, home phone number, mobile phone number, email address
  • Retention Basis: Terms of use for the hospital website / Article 15 of the Medical Service Act Enforcement Rules (Retention of Medical Records)
  • Retention Period: Destroyed upon membership withdrawal / Medical records retained for 10 years

Furthermore, even after the purpose of collection or provision has been achieved, personal information may be retained if required by provisions of laws such as the Commercial Act or other relevant regulations.

 

Article 7: Personal Information Destruction Procedure and Method

The Company, in principle, destroys personal information immediately after the purpose of its collection and use has been achieved. The destruction procedure and methods are as follows:

Personal information stored in electronic file formats is deleted using technical methods that ensure the data cannot be restored. Personal information printed on paper is destroyed either by shredding with a shredder or by incineration.

 

Article 8: Rights of Users and Legal Representatives and Their Exercise Methods

The Company sincerely responds to and promptly handles user requests to access, correct, or delete personal information. However, to protect personal information, the Company does not provide procedures for access, correction, or deletion through methods such as phone, mail, or fax other than the user's in-person visit.

  • Users may request access to their personal information by visiting the Company, and the Company will respond promptly to such requests.

Correction and Deletion of Personal Information

  • If a user requests correction or deletion of their personal information and it is determined that there is an error or a need for correction or deletion, the Company will process the request without delay. The Company may request supporting documents necessary to verify the facts related to the requested correction or deletion.
  • When a user requests access, correction, or deletion of their personal information, the Company will confirm the user's identity by requiring identification documents such as a resident registration card, passport, or driver's license.
  • If the Company has a legitimate reason to deny access, correction, or deletion of all or part of the personal information, it will notify the user and explain the reason.
  • A legal representative of a child under the age of 14 may request access, correction, deletion, or suspension of the processing of the child’s personal information by submitting proof of their relationship with the child and their identification.

※ Personal information that is legally required to be retained cannot be corrected or deleted during the mandatory retention period, even if requested.
 

Article 9: Withdrawal of Consent / Account Deletion

You may withdraw your consent to the collection, use, and provision of personal information provided during membership registration at any time. To withdraw consent (delete your account), you can proceed by clicking "Account Deletion" in the "My Page" section of the Company’s website and completing the identity verification process. Alternatively, you may contact the personal information officer via written request, phone, or fax. The Company will promptly take the necessary measures, such as destroying your personal information.

 

Article 10: Installation, Operation, and Rejection of Automatic Personal Information Collection Devices

The Company uses cookies to provide users with specialized, customized services. Cookies are small pieces of information that a website sends to a user's web browser and may be stored on the user’s PC hard drive.

The Company uses cookies for the following purposes:

  • To analyze the access frequency or visit times of members and non-members and to identify user preferences and interests for service improvement.
  • To gather data on visit counts during various events hosted by the Company to provide differentiated information tailored to individual interests.

Users have the option to manage cookie settings. By adjusting the options in your web browser, you can allow all cookies, be notified whenever a cookie is stored, or refuse the storage of all cookies. Please note that refusing cookies may result in difficulties in using certain services.

 

Article 11: Personal Information Protection Officer

The Company has implemented technical measures, such as security devices, to protect users' personal information. All information provided by users is securely protected and managed using security equipment such as firewalls.

Additionally, as part of the Company’s administrative measures for personal information protection, procedures for accessing and managing users' personal information have been established. The number of personnel handling personal information is minimized, and ongoing security training is provided.

Furthermore, the Company assigns designated users to operate systems that process personal information, grants user-specific passwords, and regularly updates these passwords to ensure the security of the systems.

 

Article 12: Obligation to Notify of Policy Changes

Changes to the Personal Information Processing Policy may occur due to changes in related laws or internal operational guidelines. When such changes occur, they will be announced on the Company’s website (https://www.ppeum.com).

Article 5: Methods and Locations for Accessing Personal Video Information

  • Access Method: You may access video information by contacting the Video Information Management Officer in advance and visiting the clinic.
  • Access Location: Clinic's server room

 

Article 6: Measures for Data Subjects' Requests to Access, Confirm, or Delete Video Information
Data subjects may request to access, confirm the existence of, or delete personal video information at any time by contacting the operator of the video information processing system. However, this is limited to personal video information that captures the data subject or is clearly necessary for the urgent protection of the life, body, or property of the data subject.

The Company will take the necessary measures without delay upon receiving a request for access, confirmation, or deletion of personal video information.

However, requests to access, confirm, or delete personal video information may be denied in the following cases. If denied, the reasons for the refusal and the procedures to object will be provided to the data subject in writing within 10 days.

  • If the retention period of personal video information has expired and the information has been destroyed
  • Other legitimate reasons that justify denying the data subject's request

The video information processed by the Company is managed securely through encryption and other measures. The Company also implements administrative measures to protect personal video information, such as granting differential access rights to personal information and recording and managing details such as the date and time of video creation, viewing purpose, viewer, and viewing date and time to prevent alteration or falsification of personal video information.

In addition, physical security measures, such as the installation of locking devices, are implemented to ensure the safe storage of personal video information.

 

Article 8: Changes to the Personal Information Handling Policy
If there are additions, deletions, or modifications to the content due to changes in laws, policies, or security technologies, the changes, along with their reasons, will be announced on the clinic's website at least 7 days before their implementation.
